SCHEDULE OF VALUES, ALTERNATES, UNIT PRICES

PART 1 – PRICING 1.1 ( LUMP SUM )AMOUNT: A. Lump-sum : Includes Fixed costs, productivity factors, labor hour costs, and all mark-ups to include all Contract Documents in the amount of: One Million Eighty Eight Thousand Five Hundred and Thirty Six Dollars and 00/100 Cents; And, in U. S. dollars: $ 1,088,536.00 B. Insurance Program. 1. CCIP Program: Scope of Work to be enrolled in program. Subcontractor’s Price includes enrollment in the CCIP program as detailed in Exhibit G and referenced G-1 Insurance Manual. 2. Professional Liability Insurance: Subcontractor shall provide Professional Liability Insurance and prescribed minimum limits and period of coverage associated with professional services included in Subcontractor’s scope of work. C. Bond : Contractor’s Subcontractor Default Insurance Program (SDI). D. Sales Tax : Reference RPW Section 1.10 EXEMPTION FROM NEW YORK STATE SALES TAXES and applying requirements of Form ST-124, Contractor Exempt Purchase Certificate (Exhibit PS- 05). 1.2 M/WBE PARTICIPATION: Subcontractor acknowledges it has reviewed M/WBE participation goals for the Project and will meet the base requirements stated in Exhibit T. Subcontractor is committing assignments where certified M/WBE = 100% of the Subcontract Amount. Actual will be documented, tracked, and reported in accordance with Contract Documents. 1.3 PROJECT LABOR AGREEMENT (PLA ): Subcontractor understands and acknowledges that Contractor ( Skanska/Walsh Joint Venture (“SWJV”) ) and the New York City Building and Constructions Trades Council and its Affiliated Unions have negotiated a Project Labor Agreement (“PLA”). The Project Labor Agreement can be found with the Contract Documents and is referenced in Exhibit PS-08. Subcontractor agrees, for itself and all lower tier subcontracts or agreements as the PLA shall require, to be bound by the terms and conditions of the final, fully executed PLA, as the same shall be amended from time to time. 1.4 SCHEDULE: Project Schedule Attachment Subcontractor acknowledges Project Schedule Exhibit and will meet all start dates, durations and completion dates required for the Work to maintain this Schedule. 1.5 ADDENDA: All addenda included herein have been included in this Subcontract Agreement and are listed
