LULL Usage: BC 02 and BC 10 – Lull Use October through April SWJV’s “Magni Tracker” form captured daily Operator hours by company, including a company representative’s signature a field, verifying agreement of Lull Operator hours. SWJV’s documentation was sporadic , sloppy and at times completely nonexistent. In the absence of the contemporaneous documentation that included verification of hours by signature, SWJV provided a “Lull Usage Tracker Summary.” The supplied Lull Usage Tracker document is simply a monthly summary of Lull Operator hours and often filled with errors. SWJV would never accept a subcontractor Field Work Order, or T&M Ticket (“Ticket”) without an authorized SWJV representative’s signature verifying labor hours on the Ticket. Likewise, Spearhead will not accept documentation of Lull Operator hours without a Spearhead representative’s signature as verification of Operator hours. Out of 106 total hours attributed to Spearhead, only 30% of those hours contain a Spearhead employee signature verifying Lull Operator hours. These BBHs were installed prior to roof membrane from on top of roof while bridge concrete decking cured. Spearhead coordinated the call between the manufacturer’s Engineer and HOK’s Engineer, directly. HOK attributed $338 to this product approval.
